"Process 4" Items in grey are being created or corrected so just work on items 1. 2. 3.& 4. for right now, 5 & 6 later some documents that once were here have been set aside for now or moved to later process These
are the documents Eddie asks you to provide.
Try to have them ready for next filing by Jan 22, 2008. -- But do not get discourage if you are not able to meet that date, as this is an on-going action, especially adaptable for new participants. Later filings dates will occur where you will be entered. Also if you have already sent your documents based on previous instructions posted here, do not worry if you see modifications here or any other step / process you may have already completed along the way. Copyrights created before (in Process 2. 1.&2.) -- add your information as indicated in all areas highlighted in yellow : also add the statement hightlighted in green exactly as it is shown here -- then (for this subsequently "Amended"

the "amended'' areas (#3 & #4 of the TX from) that are highlighted in orange, with the new date infomation (now to be the date of your birth) and mark added Xs into the check boxes as shown in the instruction example form below.
You are to discard and ignore the instructions page (per Caution noted above) and only use the blank TX form (which is page 2 - the bottom page) of the interactive PDF blank form provided.
